Membership Applications

We have two catagories of entry level membership. 

1. Regular membership to the organization is for those people who are at least 18 years old and wish to ride the ambulance on a regular basis.  You join the organization as a probationary member.  You must successfully complete the requirements of probationary member.  You are then presented to the regular membership to be voted into full active duty.  Download the regular membership application below, complete all sections and mail it to the address below to begin your exciting experience of becoming an active duty member of the organization.
2. We have a youth corp group that are for those between the ages of 14 and 19 that wish to become involved with our organization but are not 18 years of age.  This group begins learning the skills required to become an active riding member and participates in various functions to support the organization.  As a member of this organization you may be accepted by a regular riding crew to ride along an participate in actual calls.  Download the youth corp membership application below.  Complete all sections and mail to the address below.
